8/15/2021: Florida hospital officials are seeing an influx of young, healthy adults filling their wards across the state, many requiring oxygen. In the past week in Florida, 36% of the deaths occurred in the under-65 population, compared with 17% in the same week last year when the state was experiencing a similar COVID surge. Florida is the national leader in coronavirus deaths, averaging more than 150 a day in the past week.

A new analysis shows that the country is on track to pass spring’s grimmest record.
The chart you're looking at is as simple as it is disturbing. It shows that, as we should expect, far fewer people are dying, per reported cases, than in the early days of the pandemic. But on the chart below, look at August, where much of the published research on death rates ends. The improvement stops.
The numbers in the first week of August are not much different from the numbers in the first week of November. By Bedford’s method, the lagged case-fatality rate has averaged 1.8 percent since August.
This should be an extremely urgent signal that the U.S. response to COVID-19 has put the country on a dangerous track.
- These estimates were prepared by Trevor Bedford, a genomic epidemiologist at the Fred Hutchinson Cancer Research Center, in Seattle, using data from the COVID Tracking Project at The Atlantic, which compiles the cases and deaths that states report. They were then independently analyzed by the forecasting expert Ryan Tibshirani at the Delphi Group at Carnegie Mellon, which works closely with the CDC on disease modeling. If we look back over the past several months, the method Bedford used has proved more accurate than other means of forecasting near-term deaths.

The World Health Organization (WHO) recommendations for social distancing of 6 ft and hand washing to reduce the spread of SARS-CoV-2 are based on studies of respiratory droplets carried out in the 1930s. These studies showed that large, ~100 μm droplets produced in coughs and sneezes quickly underwent gravitational settling (1). However, when these studies were conducted, the technology did not exist for detecting submicron aerosols.
As a comparison, calculations predict that in still air, a 100-μm droplet will settle to the ground from 8 ft in 4.6 s whereas a 1-μm aerosol particle will take 12.4 hours (4). Measurements now show that intense coughs and sneezes that propel larger droplets more than 20 ft can also create thousands of aerosols that can travel even further (1). Increasing evidence for SARS-CoV-2 suggests the 6 ft WHO recommendation is likely not enough under many indoor conditions where aerosols can remain airborne for hours, accumulate over time, and follow air flows over distances further than 6 ft (5, 10).
Traditional respiratory disease control measures are designed to reduce transmission by droplets produced in the sneezes and coughs of infected individuals. However, a large proportion of the spread of coronavirus disease 2019 (COVID-19) appears to be occurring through airborne transmission of aerosols produced by asymptomatic individuals during breathing and speaking (1–3). Aerosols can accumulate, remain infectious in indoor air for hours, and be easily inhaled deep into the lungs. For society to resume, measures designed to reduce aerosol transmission must be implemented, including universal masking and regular, widespread testing to identify and isolate infected asymptomatic individuals

"As an infectious disease epidemiologist (albeit a junior one), I feel morally obligated to provide information on what we are seeing from a transmission dynamic perspective and how it applies to the social distancing measures. Like any good scientist, I have noticed two things that are either not well articulated or not present in the “literature” of online media. I have also relied on other infectious disease epidemiologists for peer review of this piece.
Specifically, I want to make two aspects of these distancing measures very clear and unambiguous.
First, we are in the very infancy of this epidemic’s trajectory. That means that even with these measures in place, we will see cases and deaths continue to rise globally, nationally, and in our own communities. This may lead some to think that the social distancing measures are not working. They are. They may feel futile. They aren’t. You will feel discouraged. You should. This is normal in chaos. This is the normal epidemic trajectory. Stay calm.
The enemy we are facing is very good at what it does; we are not failing. We need everyone to hold the line as the epidemic inevitably gets worse. This is not an opinion. This is the unforgiving math of epidemics for which I and my colleagues have dedicated our lives to understanding with great nuance, and this disease is no exception. Stay strong and in solidarity knowing that what you are doing is saving lives, even as people continue getting sick and dying. You may feel like giving in. Don’t.
You should perceive your entire family to function as a single individual unit: if one person puts themselves at risk, everyone in the unit is at risk.
Second, although social distancing measures have been (at least temporarily) well received, there is an obvious-but-overlooked phenomenon when considering groups (i.e. households) in transmission dynamics. While social distancing decreases contact with members of society, it of course increases contact within a group (i.e. family). This small and obvious fact has surprisingly profound implications on disease transmission dynamics. The basic mechanics of this mathematical principle dictate that even if there is only a little bit of additional connection between groups (i.e. social dinners, playdates, unnecessary trips to the store, etc.), the epidemic likely won’t be much different than if there was no measure in place. The same underlying fundamentals of disease transmission apply, and the result is that the community is left with all of the social and economic disruption but very little public health benefit.
You should perceive your entire family to function as a single individual unit: If one person puts themselves at risk, everyone in the unit is at risk. Seemingly small social chains get large and complex with alarming speed. If your son visits his girlfriend, and you later sneak over for coffee with a neighbor, your neighbor is now connected to the infected office worker that your son’s girlfriend’s mother shook hands with. This sounds silly, it’s not. This is not a joke or hypothetical. We as epidemiologists see it borne out in the data time and time again. Conversely, any break in that chain breaks disease transmission along that chain.
In contrast to hand-washing and other personal measures, social distancing measures are not about individuals, they are about societies working in unison. These measures also require sustained action before results are evident. It is hard (even for me) to conceptualize how on a population level ‘one quick little get together’ can undermine the entire framework of a public health intervention, but it can. I promise you it can. I promise. I promise. I promise. You can’t cheat it. People are already itching to cheat on the social distancing precautions just a “little”- a short playdate, a quick haircut, or picking up a needless item from the store. From a transmission dynamics standpoint, this very quickly recreates a highly connected social network that undermines much of the good work our communities have done thus far.
This outbreak will not be overcome in one grand, sweeping gesture, but rather by the collection of individual choices we make in the coming months. This virus is unforgiving to unwise choices. As this epidemic continues, it will be easy to be drawn to the idea that what we are doing isn’t working and we may feel compelled to “cheat” with unnecessary breaches of social distancing measures. By knowing what to expect, and knowing the critical importance of maintaining these measures, my hope is to encourage continued community spirit and strategizing to persevere in this time of uncertainty."

Let’s talk about the fact that both the attack rate and the death rate among the young is indeed very low with COVID19, unlike most prior pandemics. And let’s speculate about some of the biological reasons. 1/
Kids suffer so much from infectious disease around the world. It’s their leading killer (https://thelancet.com/journals/lancet/article/PIIS0140-6736(16)31593-8/fulltext… ). And so I find it haunting and sweet that they might escape the predations of COVID19. 2/
Let’s define some terms. The “attack rate” is the probability that a person will come to be infected during an epidemic. Even for COVID19, it is unlikely to be >40% in the end (i.e., within the next three years); maybe just 20% of us will get it, eventually. 3/
The “infection fatality rate” (IFR) is the probability a person will die if infected. The “(symptomatic) case fatality rate” (sCFR or CFR) is the probability of death, given that one comes to medical attention (or has symptoms). 4/
In most respiratory disease pandemics, both the very young and the very old are likely to die, resulting in a U-shaped function with age. The 1918 pandemic famously had a W-shaped function (where working-age adults also died). COVID19 is different: curve is flat at young ages. 5/
From early on, it was clear COVID19 spared children. In Wuhan, no children tested positive between November 2019 and mid-January, per an early report https://who.int/docs/default-source/coronaviruse/who-china-joint-mission-on-covid-19-final-report.pdf… 6/

Something similar, in terms of relatively benign course in kids, was observed in the 2003 SARS pandemic (a similar, albeit much more deadly, pathogen – CFR of ~10%), as well. Of course, with 100,000’s of people infected in USA, there will be some cases of young people dying. And they will get news coverage. 21/
The total US population of people <24 is about 104,000,000. With an attack rate of 7.4% and a CFR of ~0.04%, this means that about 3,000 young Americans will die of COVID19. 22/

Or could it be that “ACE2 receptors” (a protein on our cells that the virus binds to) are more immature in the lungs of the young, and so they provide less of a target for SARS-CoV-2 to enter our cells (I think this unlikely). 28/

To put estimates of COVID-19 mortality into perspective, we estimate age-specific mortality for an epidemic claiming for illustrative purposes 1 million US lives, with results approximately scalable over a broad range of deaths. We calculate the impact on period life expectancy (down 2.94 y) and remaining life years (11.7 y per death). Avoiding 1.75 million deaths or 20.5 trillion person years of life lost would be valued at $10.2 to $17.5 trillion. The age patterns of COVID-19 mortality in other countries are quite similar and increase at rates close to each country’s rate for all-cause mortality. The scenario of 1 million COVID-19 deaths is similar in scale to that of the decades-long HIV/AIDS and opioid-overdose epidemics but considerably smaller than that of the Spanish flu of 1918. Unlike HIV/AIDS and opioid epidemics, the COVID-19 deaths are concentrated in a period of months rather than spread out over decades.

To contain the global spread of the 2019 novel coronavirus epidemic (COVID-19), border control measures, such as airport screening and travel restrictions, have been implemented in several countries. Our results show that these measures likely slowed the rate of exportation from mainland China to other countries, but are insufficient to contain the global spread of COVID-19. With most cases arriving during the asymptomatic incubation period, our results suggest that rapid contact tracing is essential both within the epicenter and at importation sites to limit human-to-human transmission outside of mainland China.
The novel coronavirus outbreak (COVID-19) in mainland China has rapidly spread across the globe. Within 2 mo since the outbreak was first reported on December 31, 2019, a total of 566 Severe Acute Respiratory Syndrome (SARS CoV-2) cases have been confirmed in 26 other countries. Travel restrictions and border control measures have been enforced in China and other countries to limit the spread of the outbreak. We estimate the impact of these control measures and investigate the role of the airport travel network on the global spread of the COVID-19 outbreak. Our results show that the daily risk of exporting at least a single SARS CoV-2 case from mainland China via international travel exceeded 95% on January 13, 2020. We found that 779 cases (95% CI: 632 to 967) would have been exported by February 15, 2020 without any border or travel restrictions and that the travel lockdowns enforced by the Chinese government averted 70.5% (95% CI: 68.8 to 72.0%) of these cases. In addition, during the first three and a half weeks of implementation, the travel restrictions decreased the daily rate of exportation by 81.3% (95% CI: 80.5 to 82.1%), on average. At this early stage of the epidemic, reduction in the rate of exportation could delay the importation of cases into cities unaffected by the COVID-19 outbreak, buying time to coordinate an appropriate public health response.

You call a friend and arrange to meet for lunch. It’s unseasonably springlike, so you choose a place with outdoor seating, which seems like it should be safer. As usual, you take all reasonable precautions: You use hand sanitizer, sit a good distance from other customers, and try to avoid touching your face, though that last part is hard. A part of you suspects that this whole thing might be overblown.
What you don’t know is that ten days ago, your friend’s father was a guest of his business partner at the University Club, where he caught the novel coronavirus from the wife of a cryptocurrency speculator. Three days after that, he coughed into his hand before opening the door of his apartment to welcome his son home. The saliva of COVID-19 patients can harbor half a trillion virus particles per teaspoon, and a cough aerosolizes it into a diffuse mist. As your friend walked through the door he took a breath and 32,456 virus particles settled onto the lining of his mouth and throat.
Viruses have been multiplying inside his body ever since. And as he talks, the passage of his breath over the moist lining of his upper throat creates tiny droplets of virus-laden mucus that waft invisibly into the air over your table. Some settle on the as-yet-uneaten food on your plate, some drift onto your fingers, others are drawn into your nasal sinus or settle into your throat. By the time you extend your hand to shake good-bye, your body is carrying 43,654 virus particles. By the time you’re done shaking hands, that number is up to 312,405.

Some of the earliest insights into how COVID-19 can act like a vascular disease came from studying the aftermath of the most serious infections. Those reveal that the virus warps a critical piece of our vascular infrastructure: the single layer of cells lining the inside of every blood vessel, known as the endothelial cells or simply the endothelium.
Dr. William Li, a vascular biologist, compares this lining to a freshly resurfaced ice skating rink before a hockey game on which the players and pucks glide smoothly along.
"When the virus damages the inside of the blood vessel and shreds the lining, that's like the ice after a hockey game," says Li, a researcher and founder of the Angiogenesis Foundation. "You wind up with a situation that is really untenable for blood flow."

In order to find which cells might be the gateways for the virus, Cambridge researchers turned to the Human Cell Atlas. The atlas, launched in 2016, acts as a kind of map for the hundreds of different kinds of cells in your body, their characteristics, the genes they contain, and their relationship to other cells.
The coronavirus works by attaching its spike protein to a particular receptor on the outside of human cells referred to as ACE2 and a protein in cell membranes named TMPRSS2 which primes those receptors for the virus to enter. The cells with the highest levels of ACE2 and TMPRSS2 turned out to be in the nose — goblet cells that make mucus and ciliated cells, tiny cells that look like miniature wheat fields which clean stray bits of snot and foreign organisms out of your airways.
While the highest levels of ACE2 and TMPRSS2 were found in cells in the nose, the Cambridge team also found high levels in cells associated with other organs. Cells in the human cornea and intestines also had plenty of the coronavirus-friendly proteins

How the virus attacks the heart and blood vessels is a mystery, but dozens of preprints and papers attest that such damage is common. A 25 March paper in JAMA Cardiology documented heart damage in nearly 20% of patients out of 416 hospitalized for COVID-19 in Wuhan, China. In another Wuhan study, 44% of 36 patients admitted to the ICU had arrhythmias.
The disruption seems to extend to the blood itself. Among 184 COVID-19 patients in a Dutch ICU, 38% had blood that clotted abnormally, and almost one-third already had clots, according to a 10 April paper in Thrombosis Research. Blood clots can break apart and land in the lungs, blocking vital arteries—a condition known as pulmonary embolism, which has reportedly killed COVID-19 patients. Clots from arteries can also lodge in the brain, causing stroke. Many patients have “dramatically” high levels of D-dimer, a byproduct of blood clots

In a comparison of very sick patients in China, shortness of breath began around the seventh day of their illness, with sepsis starting on the ninth or 10th day, followed by ARDS and then ICU admission on the 12th day, said Babik. The heart and kidneys showed injury on the 15th day. On the 17th day, secondary infections began to set in. Patients died, on average, on the 19th day.
Most worrisome: As the disease progresses, ventilators aren’t saving people. Despite our best efforts, COVID-19 illness is far more lethal than traditional ARDS, claiming not 40% of victims but 70% or even 80%.

1) This is a horrible, relentless, nasty disease in its severe form. If the patient is obese and/or has renal failure, the prognosis is extremely grim. They are NOT dying from comorbidities. They are dying from COVID -19.
2) When I say relentless; most patients make some improvements initially (days 4-6), but decompensate with a 2nd cytokine storm (as many have described). It is this 2nd decompensation that is deadly and that we need to prevent.
3) We are forced to share dialysis circuits given the high percentage of renal failure (these people had normal kidneys before!) Why isn’t there more press for this fact? We need more machines to manage our patients effectively.
4) This IS ARDS full stop. While certain pts may be phenotypically different and benefit from lower PEEP, others de recruit quickly. Different phenotypes should be treated as such. We are seeing more atypical presentations than usual because the volume is tremendous.
5) My interest is in thrombosis and there is a definite hypercoagulable state associated with this illness. Anticoagulation is all over the place at different centers, but there is no randomized data yet to guide any of it (working on it).
6) Every single ICU patient (barring QTc issues) got hydroxychloroquine and it doesn’t seem to have helped. Maybe it helps for prophylaxis or in mild disease, but doesn’t appear to do anything once in ICU. It is NOT a pancea and should not be given indiscriminately.
7) Listen to people who have been studying viruses and ARDS before all this began and will continue to investigate/clinically deal with it long after we have all moved on. #COVID19 #DrFauci
8) Be skeptical, but armchair conspiracy theories without any viable solutions are extremely disheartening. We are all in this together, help us generate the data that will answer those questions you are skeptical about and more importantly help our patients.

Close observers of coronavirus studies hailed the German team’s work. There’s “huge news” in the Nature paper, David Ostrov, a professor in the Department of Pathology, Immunology and Laboratory Medicine at the University of Florida College of Medicine, told The Daily Beast.
SARS-CoV-2 starts replicating in the throat, not the lungs. For that reason, a simple throat swab is enough to test for the virus. There’s probably no need for an intrusive, unpleasant nasal swab.
There’s bad news in the German study, too.
The antibodies our bodies produce in response to COVID-19 infection don’t actually destroy this virus. In that way, it is a lot like HIV.
The antibodies are not effective at clearing the virus,” Ostrov told The Daily Beast. “This is relevant when thinking about viruses and vaccines. HIV also stimulates production of antibodies that fail to clear the virus, as do many other viruses, such as hepatitis virus C.”
“People have tried and failed to generate vaccines against such viruses, so we should not be overconfident that a vaccine strategy will work,” Ostrov added.

I spent my first few days in the hospital in stable condition, receiving supplemental oxygen. But on the evening of March 20, I took a turn for the worse. Late that night, I learned that I would need to be intubated, or placed on a ventilator.
This terrified me. A few days earlier, after my admission to the hospital, my physician father had warned me: “You better not get put on a ventilator. People don’t come back from that.”
As a patient whose life was saved by a ventilator, I believe it is an outrage and an embarrassment that a nation as wealthy as ours is even discussing possible ventilator shortages. Thankfully the United States has managed to avoid widespread rationing partly due to ventilators being sent from places of low need to places of high need. We need to make sure that every patient who needs a ventilator can get one so that as many of them as possible can survive.
This is especially true because survival rates of ventilated patients are low. My father’s dark comment reflected a grim reality: 40 percent to 50 percent of patients with severe respiratory distress do not survive being on ventilators. In New York City, where I was hospitalized, 80 percent or more of coronavirus patients who end up on ventilators have died.

Ivermectin is an FDA-approved anti-parasitic drug that has also been shown to be effective in vitro against a broad range of viruses including HIV, Dengue, Influenza and Zika virus.
While shown to be effective in the lab environment, Ivermectin cannot be used in humans for COVID-19 until further testing and clinical trials have been completed to confirm the effectiveness of the drug at levels safe for human dosing.
The wholesale cost in the developing world for the tablets is about US $0.12 for a course of treatment. In the United States, the costs is less than US$50.
So, the drug would be only 400x as expensive here, half of that plundered by Health Insurance Industrial Complex parasites.

VirScan promises to help Elledge and colleagues identify which parts of the virus the immune system responds to.
Recent work from his group suggests that people all over world infected with a particular virus make antibodies against the same proteins — “even the same amino acids” — on that virus, Elledge said.
That’s surprising, considering how many epitopes viruses have and how many antibodies are in the body’s arsenal, said Elledge. The findings led him to suspect that some epitopes are, in effect, decoys, and therefore, that not all antibodies have the desired neutralizing effect.
“The immune system may be sending out all these antibodies like shooting a shotgun and hoping some of the spray will hit the target, neutralizing some critical part of the virus,” he said.
In principle, said Elledge, VirScan could indicate which epitopes are useful targets against the new coronavirus and which are just noise. Then researchers could eliminate the useless ones from vaccines they’re developing.
